Family Nurse Practitioner – Outpatient Primary Care
We are seeking a Family Nurse Practitioner (FNP) to provide high-quality, patient-centered care in an outpatient primary care setting in New York. This position is open to **entry-level/new graduate FNPs through candidates with up to 5 years of experience**.
Key Responsibilities
* Provide comprehensive primary care to patients across the lifespan, including evaluation, diagnosis, treatment, and follow-up.
* Obtain and document complete patient histories and perform physical examinations.
* Assess and manage common acute and chronic medical conditions.
* Develop individualized treatment plans based on patient needs and clinical findings.
* Order and interpret appropriate laboratory tests, diagnostic studies, and screenings.
* Prescribe medications and treatments within the scope of New York State licensure and applicable regulations.
* Provide preventive care, health education, counseling, and wellness guidance.
* Monitor patients' progress and adjust treatment plans when clinically appropriate.
* Educate patients on medications, treatment plans, lifestyle modifications, and disease management.
* Make appropriate referrals to specialists and coordinate care when necessary.
* Maintain accurate, timely, and thorough electronic medical records and clinical documentation.
* Collaborate with physicians, nurses, medical assistants, and other healthcare professionals to provide coordinated patient care.
* Follow established clinical protocols, quality standards, and applicable New York State regulations.
* Maintain patient confidentiality and comply with HIPAA and organizational policies.
* Participate in occasional Saturday coverage as required by the practice.
Qualifications
* Master's degree or higher from an accredited Nurse Practitioner program.
* Board certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
* Active New York State Nurse Practitioner license/certification or eligibility for New York licensure.
* National Provider Identifier (NPI); DEA registration preferred or ability to obtain as required.

* Strong clinical assessment, communication, and patient-care skills.
* Ability to work effectively as part of a multidisciplinary healthcare team.
* Comfortable working in a fast-paced outpatient environment.
Schedule
* Monday–Friday
* One Saturday per month
* Primary locations in Upper Manhattan, including (138th Street) & 125th Street**
* Providers may occasionally be asked to provide coverage at another practice location when needed.
